CityDoc Urgent Care has partnered with HealthMark Group to manage and facilitate medical record requests. HealthMark is a trusted partner that specializes in facilitating medical record release and keeping your personal health information safe.
How do I request my medical records?
Click the button at the top to enter your email and start the process. You will receive an email to start the records request.
Will I have to create an account?
No. Your request will be managed via email using secure links. No new account, username, or password needed.
How will I receive the requested records?
Once your request is complete, you will receive an email with a secure link to download your records.
How long does it take to process requests?
Records can be available for download in as few as 8 business hours! However, some records may require physician approval and may take up to 5 business days. If it has been more than 5 days since you submitted your request, please contact HealthMark at status@healthmark-group.com for an update.
